Corrected calcium for albumin

Adjusts total serum calcium for a low albumin, so that hypocalcemia or hypercalcemia isn't missed or overcalled.

What goes into the Corrected calcium

  • Total calcium (mg/dL)
  • Albumin (g/dL)

How to interpret the result

  • Corrected Ca = measured Ca + 0.8 × (4.0 − albumin)
  • Normal about 8.5–10.5 mg/dL; 10.5–12 mild, 12–14 moderate, > 14 severe hypercalcemia

Clinical pearls

  • The correction is unreliable in CKD, critical illness and acid-base disturbance; measure ionized calcium in those patients.
  • Symptoms and the rate of rise matter as much as the number.
