Creatinine clearance (Cockcroft-Gault)

Cockcroft-Gault estimates creatinine clearance in mL/min. Most drug labels, including DOACs and many antibiotics, base renal dose adjustments on it rather than eGFR.

What goes into the Creatinine clearance (Cockcroft-Gault)

  • Age (years)
  • Weight (kg)
  • Creatinine (mg/dL)
  • Sex: Male; Female
  • Height (optional) (cm)

How to interpret the result

  • CrCl = (140 − age) × weight ÷ (72 × creatinine), × 0.85 if female
  • Shown with actual, ideal and (if over 120% of ideal) adjusted body weight

Clinical pearls

  • Unreliable when creatinine is changing (AKI) and in very low muscle mass.
  • Check which weight your institution's pharmacy uses for dosing in obesity.
